Road Map Home for Veterans

The first few days after you get home from the hospital can be unpredictable. The risk of rehospitalization is a reality during those first days of transition. We also know that veterans want to go home and stay home.

Elara Caring’s Personal Care Services for veterans can help make this return to home a success. We can support patients transition home and stay in place as needed to support stability, independence and dignity.

Elara Caring Personal Care Services funding is typically covered by your VA Benefits but may also be covered by Medicaid, insurance or sometimes private pay. We can work with your local agencies to seek approval and evaluate your needs to determine coverages available to you. Programs vary based on your area.

I didn’t want to admit I needed help when I got home from the hospital, but having a caregiver made so much difference. I was just slower getting back to myself this time, and she helped me through the time of transition. I feel much better now and can continue living independently.
